- Must be 18 years of age, or have permission from parents if under 18 (please provide date of birth on the application).

- Write a 700-2500 word essay on the following topic: any personal injury or accident you or your loved one has experienced, and your suggestions or lessons on how to avoid or prevent such incidents, or methods and solutions to decrease the chances that such an incident will occur in the future
