A new scientific statement <a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIR.0000000000001419\" target=\"_blank\" rel=\"noopener\" title=\"\">published<\/a> in <em>Circulation<\/em>, developed by experts at Weill Cornell Medicine and other leading institutions, lays out the physiological dangers that temperature extremes pose and calls for urgent action at every level, from bedside medicine to national policy.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why Extreme Temperatures Strain the Heart<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">During a heat wave, the human body works hard to cool itself. Blood vessels widen, sweating increases, and the heart compensates by beating faster and pumping with greater force. Blood pressure drops and fluid levels fall. For most healthy people, this is manageable. But for those already living with cardiovascular disease, this cascade of physiological changes can push the heart past its limits. Complicating the picture further, many common cardiac medications \u2014 including diuretics \u2014 cause the body to shed water, adding to the fluid imbalance and increasing the risk of a cardiac event.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cold weather carries its own set of dangers. Historically, cold temperatures have been responsible for more cardiovascular deaths simply because frigid days are more frequent than extreme heat events. When temperatures drop sharply, blood vessels constrict, blood pressure rises, and the heart must work harder to maintain circulation \u2014 a combination that elevates the risk of heart attack and stroke. But the report warns that as climate change drives heat waves to become more frequent, more intense, and longer in duration, heat-related cardiac deaths could eventually surpass any reduction in cold-related mortality.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Who Is Most at Risk<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Certain groups face disproportionately high danger from temperature extremes. Older adults are especially vulnerable because the aging body loses some of its capacity to regulate internal temperature efficiently. The statement also identifies pregnant individuals, infants, outdoor workers and people with lower incomes as high-risk populations. Lower-income communities are particularly exposed \u2014 residents often live in neighborhoods with less tree canopy, more heat-absorbing pavement, and limited access to air conditioning, creating what researchers call urban heat islands.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For college students and young adults, the risks may feel remote, but outdoor workers, student athletes training in summer heat, and young people with undiagnosed heart conditions all face real exposure. Understanding what makes extreme temperatures dangerous is the first step toward staying safe.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Critical Gaps in Medical Knowledge<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">One of the statement&#8217;s most striking findings is how much physicians still do not know. Arnab Ghosh, an associate professor of medicine at Weill Cornell and an internist at NewYork-Presbyterian\/Weill Cornell Medical Center, led the development of the policy section of the report. He pointed to a fundamental gap in clinical guidance around medication management during heat events.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\">&#8220;When it&#8217;s a hundred degrees outside, should I be adjusting my patients&#8217; medications? Right now, we just don&#8217;t know,&#8221; Ghosh said in a news release.<\/p>\n<\/blockquote>\n\n\n\n<p class=\"wp-block-paragraph\">The report calls for more research to determine the precise temperature thresholds at which people with cardiovascular risk are endangered, and to better understand how repeated or prolonged heat exposure affects the heart over time. How different combinations of medications interact with heat stress is another open question the authors say demands investigation.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Health Care System&#8217;s Own Carbon Problem<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Perhaps the most counterintuitive finding in the statement is that the health care sector itself is a significant driver of the climate conditions fueling these risks. In the United States, health care accounts for roughly 8.5% of total greenhouse gas emissions. The energy demands of running MRI machines, the gases used in operating rooms, and the mountains of single-use disposable equipment all contribute to the sector&#8217;s substantial carbon footprint.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The statement&#8217;s authors argue that providers can take steps toward more conservative prescribing and reduced reliance on disposable supplies, but systemic change will require rethinking how care is delivered altogether. Ghosh pointed to telehealth as one concrete example of a lower-carbon alternative that cuts emissions from patient travel without sacrificing care quality.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\">&#8220;We need a fundamentally different system that still delivers the same health benefits but is also good for the planet,&#8221; Ghosh added.<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\">Policy and Urban Design as Medicine<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Beyond the clinic, the report urges policymakers to treat extreme heat as a public health emergency requiring infrastructure-level responses. Keeping energy costs affordable so households can run air conditioning, and establishing accessible cooling centers for those without it, are among the practical recommendations. Urban greening also plays a role \u2014 but the statement&#8217;s authors emphasize that planting trees strategically matters more than simply planting more of them.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ghosh recently <a href=\"https:\/\/www.nature.com\/articles\/s41467-026-70723-6\" target=\"_blank\" rel=\"noopener\" title=\"\">published research<\/a> in <em>Nature Communications<\/em> showing that the most effective cooling comes not from isolated trees but from dense, continuous canopies positioned close to homes, streets and public gathering spaces \u2014 the areas where people spend the most time outdoors and are most vulnerable to heat exposure.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Reducing greenhouse gas emissions remains the most effective long-term solution to limiting heat&#8217;s toll on cardiovascular health. But given that 2024 was the <a href=\"https:\/\/www.nature.com\/articles\/s41467-026-70723-6\" target=\"_blank\" rel=\"noopener\" title=\"\">hottest year on record<\/a> since the National Oceanic and Atmospheric Administration began tracking temperatures in 1880, the researchers stress that society cannot afford to wait for long-term fixes alone.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;As heatwaves increase, we need coordinated action in research, clinical practice and policy to understand how we can keep people safe,&#8221; added Ghosh. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The statement&#8217;s urgency is captured in his closing call: &#8220;Heat is a problem now; we can&#8217;t wait. We need more tools and creative solutions for protecting people.&#8221;<\/p>\n\n\n\n<div style=\"height:10px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p
