But suppressing the virus does not always mean the body fully heals. Research <a href=\"https:\/\/insight.jci.org\/articles\/view\/201258\" target=\"_blank\" rel=\"noopener\" title=\"\">published<\/a> May 22 in <em>JCI Insight<\/em> from Tulane University reveals a specific reason why gut damage can persist in people living with HIV \u2014 and points to a surprisingly accessible potential remedy found in the produce aisle.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Problem That Persists After Treatment<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">When HIV takes hold, it attacks the gut lining with particular ferocity, disrupting the intestinal barrier and triggering immune dysfunction. For decades, researchers have known that even patients on long-term antiretroviral therapy continue to experience elevated inflammation \u2014 a condition tied to higher risks of cardiovascular disease, metabolic disorders and other serious health complications. What has been less clear is exactly which immune mechanisms fail to recover, and why.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Tulane study, led by Namita Rout, an associate professor of microbiology and immunology at the Tulane National Biomedical Research Center, set out to answer those questions. Her team examined gut tissue from nonhuman primates infected with SIV \u2014 a virus closely related to HIV that is widely used as a research model \u2014 after the animals had received extended antiretroviral treatment. Despite successful viral suppression, the researchers detected clear signs of intestinal barrier dysfunction and disruption in critical immune cell populations.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Which Immune Cells Are Affected<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The study zeroed in on two types of immune cells \u2014 gamma delta T cells and innate lymphoid cells \u2014 that play central roles in maintaining the integrity of the gut lining. Under normal conditions, these cells release signaling molecules that protect intestinal tissue and coordinate repair when damage occurs. In the treated animals, those protective responses were measurably diminished and appeared connected to disruptions in proteins that help direct immune activity at the gut barrier.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This finding helps clarify a longstanding puzzle. Antiretroviral drugs are highly effective at reducing viral load, but they do not appear to fully restore the immune signaling networks that keep the intestinal environment balanced. The result is a kind of ongoing low-grade injury that conventional HIV treatment was not designed to address.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Where Broccoli Comes In<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Rout&#8217;s team then explored whether a dietary intervention could influence the damaged immune pathway. A small cohort of animals was given a broccoli-based supplement intended to elevate levels of compounds called indoles \u2014 naturally occurring molecules abundant in vegetables from the mustard family, including broccoli, cabbage and kale. After just one month of supplementation, the animals showed indicators consistent with improved gut barrier integrity, along with shifts in immune cell populations associated with mucosal repair.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The underlying mechanism involves what researchers call the AHR-ROR\u03b3t axis, a biological pathway that indoles can activate and that plays a role in regulating immune homeostasis in the gut. The study&#8217;s formal title \u2014 &#8220;Dietary Indoles Influence the AHR-ROR\u03b3t Axis and Mucosal Immune Homeostasis in ART-Treated SIV Infection&#8221; \u2014 reflects that mechanistic focus.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is important to note what the findings do not show. The study was conducted in a small number of animals, not human patients, and the results should not be interpreted as evidence that broccoli supplements are a treatment for HIV or its complications. The research represents an early but meaningful step in understanding a biological pathway that may still be amenable to intervention even after years of antiretroviral therapy.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why It Matters for Long-Term HIV Care<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">For the roughly 39 million people worldwide living with HIV, the prospect of addressing residual gut damage through dietary strategies \u2014 rather than additional pharmaceuticals \u2014 carries real appeal. Rout emphasized that the study&#8217;s value lies in illuminating a mechanism that could inform future approaches to care.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\">&#8220;This study helps us better understand why gut damage and chronic inflammation can persist even when the virus is well controlled,&#8221; Rout said in a news release. &#8220;These findings identify an immune pathway that appears to be important to intestinal health and may help guide future nutritional strategies aimed at improving long-term health outcomes for people living with HIV.&#8221;<\/p>\n<\/blockquote>\n\n\n\n<p class=\"wp-block-paragraph\">For college students and young adults who may be navigating an HIV diagnosis or supporting someone who is, findings like these underscore that the science of HIV care is still evolving well beyond controlling the virus itself. Researchers are increasingly focused on quality of life, long-term organ health, and the role that nutrition and lifestyle factors might play alongside standard treatment.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What Comes Next<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The Tulane team&#8217;s work opens the door for larger clinical investigations that could test indole-based dietary interventions directly in humans with HIV.
