The research, led by Margaret E. Kruk of Washington University School of Medicine in St. Louis, surveyed more than 32,000 adults across 18 countries and found striking, consistent gaps in how this population is served by health systems worldwide.

The study draws on data from the People’s Voice Survey, collected in 2022 and 2023 from over 1,000 respondents in each participating country — spanning high-, low- and middle-income nations. Participants self-assessed their physical and mental health, then reported on their confidence in the healthcare system, their use of services, the quality of care they received, and their sense of empowerment to manage their own health — a measure researchers call “patient activation.”

A Consistent Gap Across Borders

Regardless of country, respondents who rated their mental health as poor were more likely to also report chronic illness, lower overall health, reduced patient activation, poorer care quality, and diminished trust in healthcare institutions. The proportion of people in this group who had received mental health treatment in the past year ranged widely — from just 0.9% in Lao PDR to 52.4% in the UK — revealing dramatic disparities in access to care.

National mental health profiles also varied significantly. Nigeria had the lowest share of respondents reporting poor or fair mental health at 4.7%, while China had the highest at 39.6%.

“People with poor mental health had worse care, more unmet needs, and less trust in the system, regardless of where they lived,” Kruk, the Distinguished Endowed Professor of Health Systems & Medicine at WashU Medicine, a professor at WashU Public Health and director of the universitywide QuEST Center and the international QuEST Centers and Network, said in a news release.

The authors also highlighted that mental health struggles rarely exist in isolation. People who reported poor mental health were nearly twice as likely to have a chronic illness and far less likely to feel capable of managing their own health — suggesting that healthcare systems that treat mental health as a separate category may be missing a much larger picture.

Why It Matters for Students and Young Adults

Rates of depression and anxiety have climbed globally since the COVID-19 pandemic, and young adults have been among the hardest hit. For college students and recent graduates navigating campus health centers, insurance coverage, or mental health waitlists, this research offers data to back what many already feel: that the system often falls short for those who need it most.

The study’s findings about patient activation — the degree to which individuals feel empowered to understand and manage their own health — are particularly relevant for young people just entering healthcare systems independently. When that confidence is low, people are less likely to seek help, follow through on treatment, or advocate for themselves with providers.

What Researchers Are Calling For

While the study is descriptive in nature, the researchers see patient activation as a promising target for intervention. They suggest that improving people’s confidence and engagement in their own care could have downstream benefits for overall health and wellness — not just mental health outcomes.

The team also recommends that health systems stop siloing mental health services and instead recognize that patients with mental health conditions show up across all areas of care, often with more complex needs than average. Tracking system performance over time and comparing outcomes across similar health systems, they argue, could help drive continuous improvement.

Source: WashU Medicine

A sweeping survey across 18 countries reveals that people with poor mental health consistently experience worse healthcare quality, more unmet needs, and less trust in medical systems — no matter where they live.
